Apathya in Amavata – Things to avoid in Rheumatoid Arthritis

Treatment and medicines for a given disease is important. Very important is also the awareness and knowledge of Do’s and Don’t’s, Good’s and Bad’s with respect to diet and lifestyle. These will provide a supporting factor during the phase of recovery from the concerned illness.

In this context let me tell you what are bad, unwholesome and harmful things (Apathya) and also things to avoid while on treatment for Amavata vis-à-vis Rheumatoid Arthritis.

Aahaaraadi Rupa Apathya {Apathya in the form of Diet, Vegetables, Grains etc.}:

  • Dadhi – Curds
  • Matsya – Fish
  • Guda – Jaggery / Molasses
  • Ksheera – Milk
  • Potaki
  • Masha – Black Grams
  • Upadika –
  • Pishtakam – Flour products
  • Guru Ahara – Foods which are heavy to digest
  • Abhishyandi Ahara – Foods which cause stagnation and blocks in the body
  • Viruddha Ahara – Mutually incompatible foods
  • Vishama Ashana – Odd and weird practices of food wrt quality, quantity and timing of food
  • Dushta Neeram – Contaminated water

Vihara Roupa Apathya:

  • Poorvavata – Getting exposed to winds coming from east
  • Vegarodham – Suppressing or forcibly avoiding / neglecting the natural calls of the body like those of Flatus, Stools, Urine, Hunger, Thirst, Sleep etc.
  • Jaagaram – Excessive awakening during night and making a habit of the same for prolonged periods

उपनाहाश्च कर्तव्यास्तेऽपि स्नेहविवर्जिताः। (यो.र)

यवाः कुलत्थाः श्यामाकाः कोद्रवा रक्तशालयः।

वास्तुकं शिग्रु वर्षाभूः कारवेल्लं पटोलकम्॥

आर्द्रकं तप्तनीरं च लशुनं तक्रसंस्कृतम्।

जाङ्गलानां तथा मांसं सामवातगदे हितं॥ (यो.र)

Treatment and medicines for a given disease is important. Very important is also the awareness and knowledge of Do’s and Don’t’s, Good’s and Bad’s with respect to diet and lifestyle. These will provide a supporting factor during the phase of recovery from the concerned illness.

In this context let me tell you what are good, wholesome and beneficial things to eat and do (Pathya) while on treatment for Amavata vis-à-vis Rheumatoid Arthritis. Since Chikitsa (treatment) is also wholesome in the recovery of a disease, it will also be considered as Pathya. I shall also enlist the basic fundamental treatments (protocol) advised to be implemented in Amavata.

 Chikitsa Rupa Pathya (Wholesome and beneficial treatments):

  • Langhana: Lightening therapies especially Fasting
  • Snehana: Oleation therapy, Oral administration of Medicated lipids (oils & or ghee)
  • Swedana: Sweat inducing therapies, Sudation, Fomentation. Ruksha Sweda or Dry fomentation is preferred Ex. Valuka / Baluka Sweda (Sand Bolus Fomentation)
  • Virechana: Purgation
  • Asthapana / Kashaya Vasti: Decoction Enemas / Medicated Milk Enemas
  • Anuvasana Vasti: Oil / Ghee Enemas especially using oils of the like of Saindhava Taila
  • Kshara Vasti: Enemas predominantly comprising of Alkaline Medicines
  • Sneha-Vivarjita Upanaha: Medicinal poultices devoid of Sneha (lipids)

Oushadha Rupa Pathya (Pathya in the form of Medicines):

  • Panchakola (Herbal medicine in the form of powder prepared using 5 medicaments) is said to be the best option during the early stages of the disease. Food and water processed with Panchakola should always be given as a part of diet. Water processed with Panchakola Churna should be used every time the patient feels thirst and should be preferred instead of plain water.

{There are many formulations to relieve Amavata but we are only discussing the principle of treatment and Pathya in this context}

Aahaaraadi Rupa Pathya {Pathya in the form of Diet, Grains, Vegetables etc.}:

  • Yava – Hordeum vulgare (Barley)
  • Kulattha – Dolichos biflorus
  • Shyamaka – Echinochloa frumentacea Linn (Barnyard Millet)
  • Kodrava – Paspalum scrobiculatum Linn.
  • Raktashaali – Red variety of Rice or Paddy (Oryza sativa Linn.)
  • Vaastuka – Chenopodium album Linn. (Lambs Quarters)
  • Shigru – Moringa olifera or Moringa pterygosperma Gaertn. (Horse Radish, Drum Stick)
  • Varshabhu – Trianthema portulacastrum Linn.
  • Kaaravellaka – Momordia charantia Linn. (Bitter gourd)
  • Patolakam – Trichosanthes dioica Roxb. (Pointed gourd)
  • Ardhrakam – Zingiber officinale Rosc. (Ginger)
  • Taptaneeram – Hot or Boiled-&-cooled or Warm water
  • Takra Samskrutam Lashunam – Garlic processed with buttermilk

Ingredients: Equal parts of-

  • Rasna – Pluchea lanceolata
  • GuduchiTinospora cordifolia
  • Erandamoola – Roots of Ricinus communis (castor plant)
  • Devadaru – Cedrus deodara
  • Shunti – Zingiber officinalis

Rasnapanchakam Kashayam / Rasnapanchakam Kwatham is a simple yet effective formulation used in combating Amavata vis-à-vis Rheumatoid Arthritis. It is known to eradicate the disease pathology apart from giving good relief from pain, swelling and associated conditions.

As the name indicates Rasnapanchakam (panchakam=five ingredients) comprises of 5 drugs including Rasna.

Form: The medicine should be used in the form of decoction

Method of Preparation:

The above said herbs are taken in their dry form in equal proportions. The drugs are measured after pounding them into coarse powder and not in the raw and available form. Each drug is individually measured.

16 parts of water is added (Combination of drugs 1 part : Water 16 parts) to the ingredients and the contents are boiled on moderate flame until ¼ of it remains. Filter and separate the hard part. The liquid filtrate is called Kashaya or Kwatha. It is preferable to use it hot and fresh.

Note: The quantity of ingredients and water will be changed depending on the nature & strength of the disease, dosha’s and the diseased

Indications:

  • Amavata – Rheumatoid Arthritis
  • Sandhigata Vata – Osteoarthritis
  • Sarvanga Vata – Generalized body pains, Myalgia, Musculo-skeletal pain
  • Asthigata Vata – Bony pains, Osteoporosis

Dose:

  • 40-50ml twice to thrice daily, preferably before food

दशमूलं हरीतक्या तुल्यं कंसहरीतकी।

मानं तेनात्र तत्रस्थं चरके प्राह जेज्जटः॥ {यो.र.शोथ.चि}

Kamsa Haritaki is an unparalleled herbal compound prescribed for relief from Oedema, Swelling, Anaemia and many other health conditions. It is a herbal laxative useful in these conditions for elimination of metabolic wastes and accumulations, cleanse the channels and pathways of body and detoxify the system. Thus it relieves many diseases. Haritaki or Terminalia chebula is the chief ingredient of this formulation.

Ingredients:

Dw i-Panchamoola / Dashamula / Dashamoola (10 roots – mentioned below): Each is taken in equal proportion and the total quantity should sum up to 1 Adhaka (4kgapp r) quantity.

  • Bilwa – Aegle marmelos
  • Agnimantha – Clerodendrum phlomidis / Premna mucronata (obtusifolia)
  • Shyonaka – Oroxylum indicum
  • Patala – Stereospermum suaveolens
  • Gambhari – Gmelina arborea
  • Shalaparni – Desmodium gangeticum
  • Prishniparni – Uraria picta
  • Brihati – Solanum indicum
  • Kantakari – Solanum surattense / Solanum xanthocarpum
  • Gokshura – Tribulus terrestris

Jala: Water – 1 Drona quantity (1024 tola) – 12288ml

Haritaki – Fully ripened fruits of Terminalia chebula – 1000 number

Purana Guda – Old Jaggery – 100 pala (4800 grams appr)

Trikatu – 1 pala each (48 grams each):

  • Pippali – Piper longum
  • Maricha – Piper nigrum
  • Shunti – Zingiber officinale

Trijata – 1 karsha each (12 grams each):

  • Twak – Cinnamomum zeylanicum (Cinnamon)
  • Ela – Elettaria cardamomum (Cardamom)
  • Patra – Cinnamomum tamala

Yava Kshara – 1 karsha (12 gram) – Potassium carbonate (alkali prepared with barley – Hordeum vulgare)

Madhu – Honey – ½ Prastha (500 gram) quantity.

Method of Preparation:

  • Dashamoola / Dashamula (10 roots) are taken in the said proportion and processed (boiled on moderate flame) in 1 drona of water
  • When ¼ of the taken quantity of water remains the Dashamoola Kashaya is prepared (Decoction of Dashamoola)
  • The Kashaya is filtered
  • To the filtered Kashaya the fruits of Haritaki (Terminalia chebula) – 100 numbers and Old Jaggery in 100pala quantity is added and further processed on fire till the contents attain the form of lehya (confection, jam)
  • To this herbal jam fine powders of Trikatu, Trijata and Yava Kshara in mentioned proportions are added and mixed thoroughly
  • The contents are allowed to cool
  • To this, Honey in mentioned proportion is added and mixed properly
  • The resultant formulation is called Kamsa Haritaki
  • The compound is stored in air tight containers

Dose:

  • 1 fruit of Haritaki (Terminalia chebula) should be taken per day followed by Leha (jam) in a dose of Shukti Pramana (1/2 pala) – 24 grams
  • According to Acharya Jejjata the dosage is same as Dashamoola Haritaki

Indications:

Kamsa Haritaki effectively alleviates the below said conditions:

  • Pravrudda Shvayathu: Widespread oedema / anasarca
  • Kasa – Cough
  • Jwara – Fever
  • Aruchi – Anorexia
  • Meha – Diabetes Mellitus, Urinary Disorders
  • Gulma – Abdominal tumors
  • Pleeha – Splenomegaly
  • Tridosha – Disorders caused due to aggravation of all the Doshas
  • Udara – Abdominal Disorders, Ascitis
  • Pandu Roga – Anaemia
  • Krushata – Emaciation, Debilitating disorders, Consumption
  • Amavata – Rheumatoid Arthritis
  • Raktapitta – Bleeding disorders, Phthisis
  • Amlapitta – Gastritis, GERD
  • Vivarnata – Discolouration
  • Mutradosha – Contamination (infection) of Urine
  • Vatadosha – Vitiation of Vayu
  • Shukradosha – Contamination of Semen, Sperm

Causes for Arthritis, Pain, Musculo-Skeletal Disorders, Back-Neck Pain – An Ayurveda Perspective

Ayurveda has different basics of explaining the conditions of health and disease. Arthritis, Spinal disorders and MSD’s – all are associated with pain. The nature and intensity of pain varies from one condition to the other.

Why do we get painful conditions like various forms of Arthritis or MSD’s? Let us try to see what Ayurveda has to say about it in a nut shell.

Causes for painful conditions:

1. Agnimandhya (Disturbed Core and or tissue metabolism) and formation of Ama (Metabolites or endotoxins)

  • Unless otherwise proved, the set of events leading to formation of unwanted cellular wastes (metabolites / endotoxins) caused due to a disturbed cellular metabolism resulting from a deranged core metabolic fire in gut is the common cause of many MSD’s, Various types of Arthritis and Spinal disorders. In all the cases of painful conditions, treatment is started as default with an initial plan-up of getting rid of these endotoxins. The formation of ama also predisposes for stagnation at the level of cells and hence exposing them to various types of infections.

 

2. Srotodushti & Khavaigunya:

  • Blocks or obstruction of many types leading to deficient supply of nutrients and essential elements needed for the smooth functioning of the joints. This may in turn cause localised defects and loss of local immunity for the diseases.

3. Disturbance in equilibrium of Doshas (especially abnormal increase)

 

  • Vata Joint Disorders

A pathological increase of Vata (Vata Prakopa and allied stages) causes excess dryness, roughness and degeneration of bony joints and soft tissues causing pain of variable intensity.

Other symptoms: Swelling, Crepitus, Loss of movements of the concerned joints, difficulty in walking or working etc.

Vata disturbance in the joints can cause Degenerative arthritis like Sandhigata Vata (Osteoarthritis), Vatarakta (Vata type of Crystal Arthritis or Gout), many MSD’s and Spinal disorders of Vata origin etc

  • Pitta Joint Diorders:

A pathological increase of Pitta (Pitta Prakopa and allied stages) causes excess heat and inflammation in the joints, inflammatory swelling and soft tissues causing irritating and burning pain of variable intensity.

Other symptoms: Inflammatory swelling, burning sensation, rise in temperature, redness etc in the joints (along with other inflammatory conditions in the body)

Pitta disturbance in the joints can cause Inflammatory Arthritis like Amavata or Pittaja Vatarakta (Rheumatoid Arthritis in its various stages), many MSD’s, Neuro-muscular conditions and Spinal disorders of Pitta origin etc

  • Kapha Joint Disorders:

A pathological increase of Kapha (Kapha Prakopa and allied stages) causes excess swelling (non-inflammatory) in the joints and soft tissues causing dull, heavy pain and stiffness of variable intensity.

Other symptoms: Non-Inflammatory swelling, heaviness, stiffness, etc in the joints (along with systemic disorders caused due to sluggish metabolism)

Kapha disturbance in the joints can cause Non-Inflammatory Arthritis like Amavata (Rheumatoid Arthritis variant), Neuro-muscular conditions and Spinal disorders of Kapha origin, autoimmune disorders etc

The pathological combination of 2 doshas or all the 3 can also cause many painful conditions; the symptoms will be of mixed type and proportion.

4. Dhatu Dushti:

  • The tissues are in the form of energy providing elements and offer strength and immunity at various levels. A disease process according to Ayurveda basics starts with the localisation of aggravated Doshas in the weaker Dhatus (tissues) and ends with the destruction of the tissue and defences of tissues. This is called Dhatu Dushti.

5. Mala Sanga:

  • Improper and inadequate elimination of wastes of the body, mainly Faeces, Urine and Sweat along with tissue toxins.

Other Contributory Factors:

  • Not following the rules and regulations of Dinacharya, Sadvritta (Diurnal lifestyle and principles of living) – Includes exercise, diet, stress etc
  • Not following the rules and regulations of Ritucharya (Seasonal adaptation)
  • Abhighata – Trauma, accidents

Amruta-Nagaradi Kwatha – Best for Gout, Rheumatoid Arthritis, Skin Diseases

Amruta-Nagaradi Kwatha:

अमृतानागरधान्यक कर्षत्रितयेन पाचनं सिद्दम्।

जयति सरक्तं वातं सामं कुष्ठान्यशेषाणि॥{भा.प्र.२१/४२}

 Ingredients:

Equal portions of (12 gram each)

  • Amruta (Guduchi) – Tinospora cordifolia
  • Nagara – Zingiber officinale
  • Dhanyaka – Coriandrum sativum

Method of preparation:

  • The above said drugs are pounded into coarse powder
  • 16 times water is added
  • The contents are heated on mild fire until ¼ of it remains
  • Filter and separate the liquid extract
  • Administer hot

Dose:

  • 30-45ml, before food, twice or thrice daily (follow doctor’s recommendation)

Benefits:

This Kashaya / Kwatha effectively alleviates –

  • Vatarakta – Gout
  • Amavata – Rheumatoid Arthritis
  • Kushta – Skin diseases
