Sneeze is a natural reflex coming out as a response to various stimulating and irritating stimuli. The stimuli which can cause sneeze differs from one person to the other. Continuous sneezing or regular sneezing is a symptom of some systemic disease or a local nasal disease. Similarly not sneezing at all is also abnormal. Sneezing is also a natural cleansing mechanism trough which the body tries to detoxify itself. Unwanted dust particles and pathogens are thrown out through sneeze.

Concept of sneeze in Ayurveda:

संस्पृश्य मर्माण्यनिलस्तुमूर्ध्नि विष्वक्पथस्थः क्षवथुं करोति।{चरकसंहिता चिकित्सास्थानम् २६}

According to Charaka “The vitiated vayu traversing all through the channels of the head on touching the Marmas (vital points in the nose) in the nose produce Kshavathu or Sneeze

घ्राणाश्रिते मर्मणि सम्प्रदुष्टे यस्यानिलो नासिकया निरेति।

कफानुयातो बहुशः सशब्दस्तं रोगमाहुः क्षवथुं विधिज्ञाः॥{सुश्रुत उत्तर २२}

Susruta explains the pathogenesis of Kshavathu in a similar way with some new material “When the vitiated vayu in the head rushes into the nose along with kapha and disturbs the already afflicted / debilitated Marmas it produces discharges along with sound and this condition is called Kshavathu

Symptoms of forcibly controlling or holding back the impending sneeze reflex:

मन्यास्तम्भः शिरःशूलमर्दितार्धावभेदकौ।

इन्द्रियाणां दौर्बल्यं क्षवथोः स्याद्विधारणात्॥{चरकसंहिता सूत्रस्थानम् /१६}

भवन्ति गाढं क्षवथोर्विघाताच्छिरोऽक्षिनासाश्रवणेषु रोगाः॥{सुश्रुत उत्तर ५५}

  • मन्यास्तम्भः Cervical Pain / Spondylosis / Neck Stiffness
  • शिरःशूलः Headache
  • अर्दित Facial Paralysis
  • अर्धावभेदक Hemicrania or Migraine
  • इन्द्रियाणां दौर्बल्यं Weakness or disorientation of the sense organs (Structural or functional debility)
  • शिरोऽक्षिनासाश्रवणेषु रोगाः Diseases of the head, eye, nose and ears (Susruta)

Treatment principles in Kshavathu avarodhajanya vikara / Kshavathuja Udavarta (Diseases caused due to forcible control of sneeze):

तत्रोर्ध्वजत्रुकेऽभ्यङ्गः स्वेदो धूमः सनावनः।

हितं वातघ्नमाध्यं घृतं चौत्तरभक्तिकम्॥{चरकसंहिता सूत्रस्थानम् /१७}

तीक्ष्णधूमाञ्जनाघ्राणनावनार्कविलोकनैः।

प्रवर्तयेत्क्षुतिं सक्तां स्नेहस्वेदौ शीलयेत्॥{अष्टाङ्गहृदय सूत्रस्थानम् }

  •  अभ्यङ्गः Abhyanga: Medicated massages {Abhyanga and Swedana are very important therapies in managing vitiated vayu as they are antagonistic to the nature of Vayu (&kapha)}
  • स्वेदो Swedana: Medicated steaming or Sudation
  • स्नेहस्वेदौ (vagbhata) – Also indicates the combined use of medicated massages and sudation
  • धूमः Inhalation of medicated fumes / तीक्ष्ण धूम (vagbhata) – Strong or intense medical fumigation
  • नावनः Nasal instillation of medicines
  • वातघ्नम् अन्नं  – Vata alleviating diet
  • औत्तरभक्तिकम् घृतं Consumption of medicated Ghee after the food
  • तीक्ष्ण अञ्जन (vagbhata) – Strong or intense collyriums
  • अर्कविलोकन (vagbhata) – Standing and seeing the sun

 

 

 According to Vagbhata: तीक्ष्ण धूम, तीक्ष्ण अञ्जन, अर्कविलोकन should be administered to initiate the sneeze reflex

Straight from Samhitas: Causes and Symptoms of Ardita – Facial Paralysis

What is Ardita?

अर्धे तस्मिन् मुखार्धे वा केवले स्यात्ततर्दितम्।{दृढबल}

Ardita is a disease in which there is a deviation or crookedness leading to deformity of one side of the face alone or along with one side of the body

Therefore according to Acharya Dridabala – it can be compared to

  • Facial Paralysis or
  • Facial paralysis with paralysis of one side of the body (hemiplegia)

But the reference and explanation given by Acharya Susruta points out Ardita towards a clear case of Facial Paralysis without the involvement of one side of the body

Samprapti or pathogenesis of Ardita or Facial Paralysis according to Susruta:

शिरोनासौष्ठचिबुकललाटेक्षणसन्धिगः।

अर्दयत्यनिलो वक्त्रमर्दितं जनयत्यतः। {सुश्रुत निदानम् }

The vitiated vayu when gets lodged in the head, nose, lips, chin, forehead and the joints of the eye (or joints of all the above said parts) causes severe troublesome disturbances and causes deviation of face to one side

{This is an explanation of Facial Paralysis without the involvement of the body}

अतिवृद्दः शरीरार्धमेकं वायुः प्रपध्यते।

यदा तदोपशोष्यासृग्बाहुं पादं जानु च॥

तस्मिन् सङ्कोचयत्यर्धे मुखं जिह्मं करोति।

वक्रीकरोति नासाभृललाटाक्षिहनूस्तथा॥{चरक चिकित्सास्थानम् २८/३८४०}

When the excessively vitiated vayu gets localised in one half of the body, it depletes the blood and hence causes contractures in the upper and lower limbs and knee joints. It also causes deviation of the face towards one side. It also deviates and causes crookedness in the nose, eye brows, forehead, eyes and jaws. This is called Ardita.

{This is an explanation of Facial Paralysis with the involvement of one side of the body i.e Hemiplegia with facial paralysis}

Symptoms of Ardita:

वक्रीभवति वक्त्रार्धं ग्रीवा चाप्यपवर्तते॥

शिरश्चलति वाक्सङ्गो नेत्रादीनां वैकृतम्।

ग्रीवाचिबुकदन्तानां तस्मिन्पार्श्वे वेदना॥{सुश्रुत निदानम् }

ततो वक्रं व्रजत्यास्ये भोजनं वक्रनासिकम्।

स्तब्धं नेत्रं कथयतः क्षवथुश्च निगृह्यते॥

दीना जिह्मा समुत्क्षिप्ता कला सज्जति चास्य वाक्।

दन्ताश्चलन्ति बाध्येते श्रवणौ भिध्यते स्वरः॥

पादहस्ताक्षिजङ्गोरुशङ्खश्रवणगण्डरुक्।{चरक चिकित्सास्थानम् २८/३८४०}

 Symptoms of Ardita as said by Susruta:

  • वक्रीभवति वक्त्रार्धं : Deviation of face to one side
  • ग्रीवा चाप्यपवर्तते : Crookedness or deviation of neck to one side
  • शिरश्चलति : Trembling or involuntary shaking of head
  • वाक्सङ्गो : Obstruction to speech or speech deformity
  • नेत्रादीनां वैकृतम् : Crookedness, pain and pulsations in the eyes, brows, cheeks, nose etc.
  • ग्रीवाचिबुकदन्तानां तस्मिन्पार्श्वे वेदना : Pain in the neck, chin, teeth, jaws on the side of deformity

Additional symptoms as said by Charaka:

  • वक्रं व्रजत्यास्ये भोजनं The food enters the mouth obliquely (difficulty to eat food) when the person tries to eat food
  • वक्रनासिकम् स्तब्धं नेत्रं कथयतः When the person tries to speak, his nose gets crooked or deviated to one side and eyes get stiff or fixed at a gaze
  • क्षवथुश्च निगृह्यते –  Obstruction of sneeze in spite of the impending reflex
  • दीना जिह्मा समुत्क्षिप्ता कला सज्जति चास्य वाक् When the person tries to speak the words comes with difficulty (strained and hardly audible), broken (words get broken), interrupted, unclear and comes in splits and parcels
  • दन्ताश्चलन्ति Teeth starts shaking
  • बाध्येते श्रवणौ –  Deafness of varied intensities
  • भिध्यते स्वरः Hoarseness of voice with interrupted speech

Purvaroopa or prodroma of Ardita:

The symptoms which indicate an impending Ardita / Facial Paralysis (that Ardita will be manifested in near future):

यस्याग्रजो रोमहर्शो वेपथुर्नेत्रमाविलम्।

वायुरूर्ध्वम् त्वचि स्वापस्तोदा मन्याहनुग्रहः॥

तमर्दितमिति प्राहुर्व्याधिं व्याधिविचक्षणाः॥{सुश्रुत निदानम् }

  • रोमहर्शो : Horripulations (Goosebumps)
  • वेपथुः : Tremors
  • नेत्रमाविलम् : Eyes look pale, tired and dirty (puffiness or lacklustre)
  • त्वचि स्वापः : Numbness of the skin over the face
  • तोदः : Pricking pain
  • मन्याग्रहः : Stiffness in the sides of the neck
  • हनुग्रहः : Stiffness in the lower jaw
  • पादहस्ताक्षिजङ्गोरुशङ्खश्रवणगण्डरुक् : Pain in foot, hands, eyes, legs, thighs, temples, ears and cheeks

Acharya Chakrapani commenting on Charaka Samhita clarifies the difference between Ardita (Facial Paralysis) and Ardhanga Vata (Hemiplegia). He tells that Ardita comes in bouts / spells and the episodes are not permanent in nature whereas Ardhanga Vata is more of a permanent one. Apart from this in Ardita – the deformities are seen predominantly in the face whereas we can find their absence in Ardhanga Vata.

Acharya Vagbhata in Ashtanga Sangraha explains Ardita on the similar lines of Charaka.

In brief, the Ardhanga Ghata / Vata explained by Charaka can be classified as:

  • Samanya Ardhanga Ghata / Vata: This is a paralysis in which the facial deformities are not involved. This is also called Pakshavadha or Pakshaghata. (Hemiplegia without facial symptoms)
  • Vishishta Ardhanga Ghata / Vata: This is a type of Paralysis in which the deformities are found in the face also along with a part of the body. This can be called as Ardita. (Hemiplegia with Facial Paralysis)

 Nidaana or causes of Ardita / Facial Paralysis:

उञ्चैर्व्याहरतॊऽत्यर्थम् खादतः कठिनानि वा।

हसतो जृम्भतो वाऽपि भाराद्विषमशायिनः॥{सुश्रुत निदानम् }

  • उञ्चैर्व्याहरतॊऽत्यर्थम्: – Always (excessively) speaking loudly
  • अत्यर्थं कठिनानि खादतः Always (excessively) eating food substances which are hard
  • अत्यर्थं हसतोAlways (excessively) laughing loudly
  • अत्यर्थं जृम्भतोAlways (excessively) yawning
  • अत्यर्थं भारात्Always (excessively) lifting or carrying heavy weights (over head)
  • अत्यर्थं विषमशायिनः Always (excessively) Sleeping on uneven surfaces or sleeping in undesired places (sleeping on rooftops in open air or excessive exposure of ears to heavy breeze is said to cause Bell’s Palsy whose symptoms are similar to Facial Paralysis)
