Causes for Arthritis, Pain, Musculo-Skeletal Disorders, Back-Neck Pain – An Ayurveda Perspective

Ayurveda has different basics of explaining the conditions of health and disease. Arthritis, Spinal disorders and MSD’s – all are associated with pain. The nature and intensity of pain varies from one condition to the other.

Why do we get painful conditions like various forms of Arthritis or MSD’s? Let us try to see what Ayurveda has to say about it in a nut shell.

Causes for painful conditions:

1. Agnimandhya (Disturbed Core and or tissue metabolism) and formation of Ama (Metabolites or endotoxins)

  • Unless otherwise proved, the set of events leading to formation of unwanted cellular wastes (metabolites / endotoxins) caused due to a disturbed cellular metabolism resulting from a deranged core metabolic fire in gut is the common cause of many MSD’s, Various types of Arthritis and Spinal disorders. In all the cases of painful conditions, treatment is started as default with an initial plan-up of getting rid of these endotoxins. The formation of ama also predisposes for stagnation at the level of cells and hence exposing them to various types of infections.

 

2. Srotodushti & Khavaigunya:

  • Blocks or obstruction of many types leading to deficient supply of nutrients and essential elements needed for the smooth functioning of the joints. This may in turn cause localised defects and loss of local immunity for the diseases.

3. Disturbance in equilibrium of Doshas (especially abnormal increase)

 

  • Vata Joint Disorders

A pathological increase of Vata (Vata Prakopa and allied stages) causes excess dryness, roughness and degeneration of bony joints and soft tissues causing pain of variable intensity.

Other symptoms: Swelling, Crepitus, Loss of movements of the concerned joints, difficulty in walking or working etc.

Vata disturbance in the joints can cause Degenerative arthritis like Sandhigata Vata (Osteoarthritis), Vatarakta (Vata type of Crystal Arthritis or Gout), many MSD’s and Spinal disorders of Vata origin etc

  • Pitta Joint Diorders:

A pathological increase of Pitta (Pitta Prakopa and allied stages) causes excess heat and inflammation in the joints, inflammatory swelling and soft tissues causing irritating and burning pain of variable intensity.

Other symptoms: Inflammatory swelling, burning sensation, rise in temperature, redness etc in the joints (along with other inflammatory conditions in the body)

Pitta disturbance in the joints can cause Inflammatory Arthritis like Amavata or Pittaja Vatarakta (Rheumatoid Arthritis in its various stages), many MSD’s, Neuro-muscular conditions and Spinal disorders of Pitta origin etc

  • Kapha Joint Disorders:

A pathological increase of Kapha (Kapha Prakopa and allied stages) causes excess swelling (non-inflammatory) in the joints and soft tissues causing dull, heavy pain and stiffness of variable intensity.

Other symptoms: Non-Inflammatory swelling, heaviness, stiffness, etc in the joints (along with systemic disorders caused due to sluggish metabolism)

Kapha disturbance in the joints can cause Non-Inflammatory Arthritis like Amavata (Rheumatoid Arthritis variant), Neuro-muscular conditions and Spinal disorders of Kapha origin, autoimmune disorders etc

The pathological combination of 2 doshas or all the 3 can also cause many painful conditions; the symptoms will be of mixed type and proportion.

4. Dhatu Dushti:

  • The tissues are in the form of energy providing elements and offer strength and immunity at various levels. A disease process according to Ayurveda basics starts with the localisation of aggravated Doshas in the weaker Dhatus (tissues) and ends with the destruction of the tissue and defences of tissues. This is called Dhatu Dushti.

5. Mala Sanga:

  • Improper and inadequate elimination of wastes of the body, mainly Faeces, Urine and Sweat along with tissue toxins.

Other Contributory Factors:

  • Not following the rules and regulations of Dinacharya, Sadvritta (Diurnal lifestyle and principles of living) – Includes exercise, diet, stress etc
  • Not following the rules and regulations of Ritucharya (Seasonal adaptation)
  • Abhighata – Trauma, accidents

How to achieve?

  • Know your constitution, goods and bads with the help of Ayurveda doctor
  • Have wholesome, complete and balanced food
  • Have timely food, thus aiding proper functioning of Doshas
  • Attend the impulses of physical urges as and when they occur
  • Follow the rules and regulations for diurnal adaptation (Dinacharya) and Seasonal adaptation (Ritucharya) as mentioned in Ayurveda
  • Detoxify your system by undergoing Ayurveda therapies and Panchakarma as suggested by your Ayurveda Doctor

  • Respect your appetite and have timely food
  • Monitor quality and quantity of food according to your constitution
  • Never take water when you are hungry and never take food when you are thirsty
  • Give immediate attention to any stomach or gut upsets especially loss of appetite, indigestion, bloating, tastelessness, aversion to food, nausea etc
  • Avoid outside foods and junks, untimely food
  • Attend the impulses of physical urges as and when they occur, especially those of hunger and thirst
  • Go herbal, Eat natural

Equilibrium of Dhatus – Chief tissues, the qualitative and quantitative balance of which determines a perfect health and immunity, Generate (forms of) energy, vitality and strength

How to achieve?

  • Regular exercise, Yoga etc
  • Good balanced food
  • Ayurveda therapies (preventive and curative) like Abhyanga (Herbal massages), Swedana (Various forms of herbal fomentation), Dhara (pouring of medicated oils, milk etc over the body) etc
  • Ayurvedic rejuvenators – Rasayana Chikitsa

Equilibrium in the process of timely formation and elimination of MalaFaeces, Urine, Sweat and other tissue toxins (Cleansing mechanism and detoxification of the body) which are devoid of energy, vitality and strength and do not contribute to health when they are left behind

By following the ethics of life and the above said balancing tricks as advised in Ayurveda, we can try to be in the vicinity of good health. When we are at the best of our health we can avoid many psycho-somatic disorders which includes Arthritis and other painful joint disorders, Musculoskeletal diseases / disorders (MSD) and Spinal disorders.

Effective way to avoid Arthritis and Pains – Maintain  a  Happy ‘Joint’ Affair between Body and Mind

Joints (Bony) are the junctions or meeting places of various parts and segments of our body and help us mobilize and in locomotion from place to place. We are all (man and other living beings) programmed in a similar way keeping apart our defects and weaknesses at birth. But just like us, the joints and their functions too have evolved from time to time. This evolution too might have been responsible for lack of adaptation of the joints to some movements.

Anger and Arthritis – An Ayurveda Perspective

Do you have excessive anger? Then you might be prone to Arthritis!!!!!!

Looks strange, isn’t it?

Not exactly……

We shall look at this with an Ayurveda perspective

Most of the systemic diseases of the modern day are psycho-somatic in nature. We hardly find a sole physical disorder or a sole mental disorder. Body and mind are 2 sides of the same coin. A problem at the physical level might at a later time reflect on the mind and vice versa. Anger is one of the most encountered emotions which has a serious destructive impact at a psycho-somatic level and is also a trigger for many health issues especially those of an inflammatory origin.

“KRODAT PITTAM”

According to this quotation from Ayurveda it is clear that Kroda (anger) leads to disturbance of Pitta. Pitta is the fire within our system which carries out the heat related activities in the body, mainly metabolism. Pitta is responsible for central and cellular metabolism. If pitta is in a state of equilibrium, these processes take place smoothly and we are healthy. A pathological increase or decrease of Pitta can hamper the heat regulation in the body, leading to destruction of tissues (increase of Pitta) or blocks in the cells and body channels (decrease in Pitta).

Among various factors which aggravate Pitta, Krodha or Anger is a major one. Anger is the emotion which is a part of us and is expressed very often in varied proportions. Whenever we exhibit Anger, we are disturbing the Pitta in our system. If the anger pangs get into us as a practice it gives a permanent fuel for vitiation of Pitta. The Pitta thus increased sets in an inflammatory background in the tissues and leads to inflammatory diseases. Among these inflammatory diseases, Arthritis is one big entity. Anger may trigger any inflammatory process in the body immaterial of the organs and tissues and is not limited to arthritis. It is true for Back pain, Neck Pain, Spinal Pain, Musculo-skeletal disorders etc.

Anger is not the only causative factor for Arthritis but surely one among many. It is not only a causative factor, but also a triggering factor.

Avoiding anger or volunteering an anger management is the key to its opposite factor i.e. Happiness. A constant practice may thus help the natural flow of happiness and love to all the cells in the body, including the joints. Though it may not cure your arthritis, but it will surely make you feel better as you would have blocked the pathway of fuel for inflammation.

Arthritis….It makes us crippled and dependant…

Arthritis in any form is becoming one of the worlds burning health related issue which is making majority of those suffering crippled and dependant. We feel ourselves in a helpless condition when the joints of our body which help us in locomotion and enable us to accomplish all the activities throughout our lives give way.Living at mercy and walking with support is what we dont desire to happen atleast in the middle and active part of our lives. It not only affects the individual suffering from it but also influences passively on all the dependants. Arthritis has its deadly influence on the socio-economic life and also the proffessional life of an individual. Moreover the person will be morally lost.

World Arthritis Day is celebrated worldwide on 12th of october every year to tackle this problem and also to create the awareness regarding the arthritis. The Aims of World Arthritis Day are:

*To raise awareness of arthritis in all its forms among the medical community, people with arthritis and the general public

*To influence public policy by making decision-makers aware of the burden of arthritis and the steps which can be taken to ease it

*To ensure all people with arthritis and their caregivers are aware of the vast support network available to them.

The theme for 2009 is “Let’s Work Together”

Arthritis may be of several types including Inflammatory arthritis eg.Rheumatoid Arthritis, Degenerative Arthritis eg.Osteo-Arthritis, Crystal Arthritis eg.Gouty Arthritis etc. Whatever may be the form of arthritis they produce pain and disability.

Signs and symptoms: Pain in the afflicted joint/joints, stiffness, swelling, redness, crepitus(o.a), difficulty to move the joints, etc

Prevention: Arthritis is a life style disease. We can avoid ourselves from being the victims of Arthritis when we follow a disciplined and planned approach towards our diet and way of living. Minimum exercises like walking, running, jogging, cycling etc can avoid arthritis from creeping in by strengthning the soft tissues and joints. Ayurveda too advocates regular Vyayama or exercises as an important part of life style measures. Vyayama provides karma samartya(ability to do the work properly and uninteruptedly), deeptagni(regulation of digestive process and metabolism at the systemic and cellular levels), medasaha kshaya(depletes fat stores and destroys excess fat- we know the direct relationship between obesity and arthritis), vibhakta gaatrata(good frame work of the body) and ghana gaatrata(well built proportional body).Ayurveda too emphasises Nitya Abhyanga or daily oil massage to the whole body. The joints require regular lubrication to keep working. Abhyanga rejuvenates the musculoskeletal system(jara hara), relieves exhaution(srama hara) and subsides vitiated vata. Apart from this it enhances the eye sight(drishti prasada), gives strength to the muscles, soft tissues and joints(pushti), induces good sleep(swapna), improves color and complexion of the skin(su twak) and provides stability to the body(dhaardya krit).

Arthritis and Ayurveda: Arthritis is not a new concept for Ayurvedic science. Rather we can tell that Ayurveda is the first medical science to explain arthritis and other joint related problems. We can find references of various type of arthritis which simulate with those explained in the modern texts. Arthritis in Ayurveda is a disease involving the vitiated vata dosha in one or the other form, though the other doshas-pitta and kapha are also invariably involved being disturbed in varying proportions. The main dhatus afflicted are rasarakta(blood), mamsa(muscles, tendons, ligaments) and ashti(bone and joints). When these tissues are afflicted by one or more of the above said doshas with predominance of vata arthritis get manifested. Disturbed metabolism(agni dushti), circulating endo-toxins(ama), improper food and activities(mithya ahara and vihara) etc are also the chief causes. This indicates that most of the arthritis according to Ayurveda have a systemic origin).

Types of Arthritis as per Ayurveda: Many conditions resembling various types of the modern day arthritis is available in the Ayurveda viz:

*Amavata: Where ama is involved along with vata to produce a picture resembling an inflammatory arthritis with special mention of Rheumatoid Arthritis.

*Sandhigata vata: Where the vitiated vata enters the joints and occupies them causing swelling, pain etc. This condition indicates the explanation similar to degenerative arthritis w.s.r.t Osteo-Arthritis.

Vata-Rakta: Where  the vitiated vata along with the vitiated rakta produce painful conditions of the joints mimicking the picture of crystal arthritis w.s.r.t Gouty-Arthritis.

There are many other conditions explained but the above said are important ones. The notable feature is the mention of the word vata in common in all the conditions.

Treatment of Arthritis in Ayurveda:

The treatment of arthritis can be initiated only after understanding the ailment in detail(nidana panchaka) and after a thorough examination of the patient through Dashavida pariksah(10 fold examination of the patient), Trividha(3 fold examination) and Ashta Stana Pariksha(examination of 8 essential parameters). In general the below said treatment can be implemented as per the type of the arthritis being dealt:

*Langhana(fasting and other forms of treatment which bring lightness in the body), Deepana(enhancing the digestive fire or improving the metabolism),Pachana(digesting the endotoxins and separating them from the association with morbid doshas) etc when ama is associated with the doshas in the pathogenesis of the arthritis. Eg in Amavata

*Shodana(purificatory measures including panchakarma): In the form of :

1.Virechana(purgation) – in all types of arthritis

2.Vasti(medicated enemas) – Kashaya vastis(decoction enemas) and teekshna vastis(strong enemas-evacuatory) in amavata, Ksheera vastis(medicated enemas prepared in milk base) in vatarakta, Anuvasana and matra vastis(oil or ghee enemas) in sandhigata vata etc

3.Rakta mokshana(blood letting) – In vatarakta

4. Snehapana – Intake of medicated oils and ghee – in amavata/sandhigatavata/vatarakta

*Sthanika Chikitsa(Local treatment):

1. Janu vasti(oil pooling around the knee joints in a chamber constructed around the joint using flour so as to accomodate oil) – in sandhigata vata

2.Patra pinda sweda(sudation to the painful joint using medicated leaves tied in a cloth in the form of a bolus) – in sandhigatavata

3. Shastika shali pinda sweda(sudation to the painful joint using processed rice tied in a cloth in the form of bolus) – in sandhigata vata

4. Lepa and Upanaha( annointments and poultices) – in all types of arthritis

5. Abhyangam and swedana(massage and fomentation) – in sandhigatavata

6. Vaaluka sweda(sudation to the painful joint using heated sand tied in a cloth in the form of a bolus) – in amavata

7. Parisheka(sprinkling or spraying medicated fluids) – in vatarakta

8. Sandhi pichu(keeping a cotton pad dipped in medicated oil in contact with afflicted joint and wrapping it with a bandage to keep it in position) – in sandhigata  vata

*Shamana Chikitsa: (Palliable treatment in the form of oral medication) – Some of the important medicines useful in various types of arthritis are mentioned below:

Oils for application: Balaguluchyadi tailam, BalaAshwagandadi tailam/kuzhambu, Chandanadi tailam, Dhanwantaram khuzhambu, Karpasasthyadi tailam, Karpooradi coconut oil, Kottamchukkadi tailam, Ksheerabala tailam, Madhuyashtyadi tailam, Mahamasha tailam, Murivenna, Narayana tailam, Pinda tailam, Sahacharadi tailam, Vatamardanam kuzhambu, Vatashini tailam

Oils and ghee for internal consumption: Bala tailam, Dhanwantaram tailam(3 & 101), Gandha tailam, Ksheerabala tailam(3 & 101), Sahacharadi tailam, Gulgulu tiktakam ghritam, Varanadi ghritam etc

Kashayam’s: Amrutottaram Kashayam,Balaguluchyadi Kashayam,Dhanwantaram Kashayam, Gulgulutiktakam Kashayam/Tablets, Kokilaksham Kashayam,Maharasnadi Kashayam/Tablets,Rasnadi Kashayam(small and big),
